1. Reason for Policy J. Rodger Aesthetics has a duty to be open and honest with patients when something goes wrong with their treatment or care.

2. Policy Statement and Aims To minimise the risk of complications, J. Rodger Aesthetics will fully discuss all proposed treatments, explaining potential risks and their likelihood. Patients will also be informed about alternative treatment options, along with the associated risks, to ensure informed decision-making.

3. Scope This policy applies to all service users at J. Rodger Aesthetics.

4. Procedure If an issue arises during or after treatment, J. Rodger Aesthetics will: - Inform the patient that something has gone wrong and provide a full explanation of the situation, including expected consequences. This will be done as soon as possible. - Apologise to the patient, ensuring that the explanation is clear, sensitive, and delivered in private. - Discuss both the short- and long-term effects of the incident. - Offer a solution where possible. If the issue cannot be resolved by the practitioner, the patient will be referred to an appropriate professional for further consultation and support.

5. Reporting J. Rodger Aesthetics is committed to documenting and reporting incidents where something has gone wrong with a patient's treatment. Following an incident: - The practitioner will document and reflect on the event, seeking peer review, advice, and support to manage, resolve, and learn from the experience. - If necessary, a learning and care plan will be developed to improve future patient care. - Any adverse drug reactions or medical device issues will be reported to the MHRA Yellow Card System. The date, time, and nature of the incident will also be documented in the patient's records.
